| Physical Therapy Assistant |

| Occupational Therapist |
| To provide prescribed treatments, concerned with improving or restoring patient’s ability to function independently, in accordance with the Plan of Treatment. This position reports to the Director of Nursing. |

| Certified Occupational Therapy Assistant |
| To provide treatments of occupational therapy which involve a complex and sophisticated level of modalities, designed to restore the patient to his/her maximum potential. This position reports to the Director of Nursing. |

| Medical Social Worker |
| To supervise Social Worker, BSW, and co-sign progress notes. Provide the means for meeting the medical social needs of patients. This position reports to the Director of Nursing. |
|
| Speech Therapist |
| To plan, direct, and deliver language-communication services to patients, in accordance with the Plan of Treatment. This position reports to the Director of Nursing. |
|
| Home Health Aide |
| To provide personal care, comfort measures, and environmental services in the patient’s home as outlined in the Home Health Aide Care Plan. This position reports to the Director of Nursing. |
